Nurse Practitioner/Clinical Navigator

Description: Rutland Regional Medical Center is presently looking of Nurse Practitioner/Clinical Navigator right now, this job will be placed in Vermont. Under the direction of the Medical Director and/or physician, the Nurse Practitioner/Clinical Navigator provides healthcare services to patients by following established standards, procedures and practices in their area of specialty. Obtains detailed history and performs physical or psychological examinations. Performs or orders tests and procedures, and interprets results. Administers such therapeutic procedures as injections, immunizations, suturing and wound care. Provide physicians with assistance during surgery or medical procedures. Instructs patients on prescribed therapeutic regimens, home care and health maintenance. Evaluates outcomes of direct patient care and consults with other specialists as required. Maintains required electronic medical records as to status, treatments and procedures. Provides preventative health care services to long-term patients and arranges consultations and referrals.
This position will serve as the conduit for services to provide a patient with the experience of integrated care. Facilitates patient interactions throughout the care continuum and among service lines coordinating care by guiding the patient through the care process and connecting the patient with appropriate services. Concentrates the service lineâs offerings into a one-stop shop experience and navigates the health system on the patientâs behalf. Partners with the physician, physician offices, tertiary care center, other specialties and internal RRHS departments of the healthcare delivery team to provide seamless healthcare delivery through the multiple systems, ensuring that patients and families are informed and involved throughout the process.
Minimum Education
§
Masters Degree in Nursing.
Minimum Work Experience
§
2 years of Nurse Practitioner experience in specialty area being sought (i.e., Pain Management, Orthopaedics, Emergency Medicine, Psychiatrics, Endocrinology or Oncology).
§
Clinical care continuum navigation experience.
Required Licenses/Certifications
§
Registration with the Vermont State Education Department as a Nurse Practitioner and Certified as a Nurse Practitioner by the American Nurses Credentialing Center or equivalent certifying bodies.
§
CPR Certification.
§
Unit-specific certifications as required (ACLS, TNCC, PALS, ENPC, CPI) per specific department operational framework.
