Location: Pittsford Vermont

The outcome of information gathered is used to determine the hospital database and reimbursement of hospital claims.

ESSENTIAL FUNCTIONS

Required Certifications: RHIT, RHIA, CCS or CCS-P AHIMA

Minimum Education

Equivalent to an Associate's degree in medical information technology (with college level courses in anatomy, physiology, medical terminology, ICD 9 CM coding, and prospective payment) preferred.

College level course in Anatomy & Physiology requi! red.

Minimum Work Experience

Six to twelve m! onths of experience preferred.

Eligible for designation as a RHIT, RHIA, CCS-P or CCS preferred.

Recent health information, ER or hospital financial background.

Required Skills, Knowledge, and Abilities

Demonstrate moderate knowledge of basic computer skills.*

Demonstrate strong knowledge of Medical Terminology.*

  • Requires Pre-Employment Testing
1. Code and sequence complete medical record information according to ICD and CPT coding guidelines and payer requirements.

2. Accurately assign the levels of service for ED records.

3. Use documentation to determine level of acuity and risk of mortality within each general disease process; also identifies POA codes.

4. Identify, clarify, and code all diagnoses and procedures.

5. Comprehend the documentation and the needed documentation to promote being able to determine additional diagnoses/conditions included within the bod! y of the record that can affect the severity and risk of mortality and ROM.

6. Must understand and apply official coding guidelines as published in Coding Clinic and CPT Assistant. Must know ICD-9 codes in depth, including V-codes, E-codes and traditionally insignificant diagnosis codes.

7. Understands the clinical documentation and differentiates manifestations of disease process and other symptoms. Understanding disease etiologies enables the coder to identify deficient documentation and initiate queries to capture missed opportunities for reimbursement.

8. Identifies patterns or discrepancies in documentation

9. Optimize reimbursement for both Medicare and Medicaid, utilizing the appropriate grouper and selection of CC's and major CC's. Insure all steps are taken, i.e.; ventilator review, mortality review, and referring problematic cases to the Coding Coordinator.

10. Ability to learn and incorporate grouper and coding methodo! logy changes rapidly. Coding changes occur at least annually

1! 1. Communicates with physicians to promote understanding of rule changes and to seek additional information which will impact the codes assigned and improve documentation.

12. Utilize the physician query process and identify patterns in the query process in order to resolve disruptions in the communication chain. Work with Clinical Documentation Specialist to improve documentation.

13. Contributes to on-going monitoring and internal and external auditing processes. May perform necessary audits.

14. Provides assistance in training of new employees.

15. May facilitate audits at the direction of the Chief Compliance Officer. Acts as subject matter expert to review records as needed, advise the other coders and capture data for collection purposes, pattern identification, report to documentation team and discuss with MDs, as necessary.

16. Understands medical necessity and how Interqual criteria might be used as a resource.

! 17. Identifies and reports issues/problems with coding software.

18. Ensure timeliness of data entry, within 30 days of discharge/service for inpatients, ambulatory surgery and observation records, a target completion of 5 days from discharge for ED's and within 3 days for ancillary accounts.

19. Assure timeliness & completeness of State Data Tape.

20. Reviews all complex RAC requests.

a. Check code accuracy.

b. Identify specific documentation that supports code.

21. Completes 10 credit hours of continuing education in ICD and CPT each year.

22. Attends all required safety training programs and can describe his or her responsibilities related to general safety, department/service safety, and specific job related hazards

23. Follows the hospital exposure control plans/blood borne and airborne pathogens.

24. Demonstrates respect and regard for the dignity of all patients, families, visitors, and ! fellow employees to ensure a professional, responsible, and courteous e! nvironment.

25. Promotes effective working relations and works effectively as part of a department/unit team inter- and intradepartmentally to facilitate the department's/unit's ability to meet its goals and objectives.

26. Demonstrates Commitment to Patient Safety

a. Understands and follows the organization's patient safety-related polices, procedures and protocols.

b. Demonstrate proactive approach to patient safety by seeking opportunities to improve patient safety through questioning of current policies and processes.

c. Identify and report/correct environmental conditions and/or situations that may put a patient at undue risk.

d. Report potential or actual patient safety concerns in a timely manner per the organization's protocol.

27. Acts as a Corporate Citizen:
a. Acts with integrity by adhering to policies and compliance requirements consistently and openly reports concerns to Leadership witho! ut fear of retaliation.

b. Is aware of the legal, regulatory and accreditation requirements that apply to their area.

28. Other duties as assigned.

ADDITIONAL ESSENTIAL FUNCTIONS (LEVEL II)

29. Ensure compliance through proper coding and adherence to payer requirements.

30. Investigate/obtain clarification with payers for all issues pertaining to compliance or reimbursements and develop procedures to educate and promote consistency.

31. Provide coding education and compliance education for coding customers.

32. Assist with integration of compliance coding practices into provider routines.

33. Maintains line of communication with Compliance Officer to keep him/her informed of any potential risks for exposure! related to coding and/or documentation practices.

34. Assures! accuracy of State Data Tapes.

35. Develop and implement action following coding audits; e.g. praise, publicity, education, policy changes, etc.

Shifts: Day

Salary: $20.57 - $30.43, hourly
