Primarily a clinical position with significant administrative responsibilities, the Care Coordinator is an entry level management position. The Care Coordinator is responsible for coordination of activities related to a specific patient population and the administration of direct patient care for a specific unit and shift. The Coordinator functions as a leader of multidisciplinary teams, providing and coordinating care to patients and families. Concurrent utilization and service review, consultation, patient and family education, promotion of continuity of care, preceptorship and c! ommittee work are required at this level of practice. Educa! tion:
Registered Nurse, currently licensed in Vermont.Baccalaureate Degree in Nursing preferred. Experience:
Three years of relevant RN experience
This is a general surgery, cardiothoracic surgey care coordinator
OR Nursing Experience Required
